This happened last month at a carnival in the Philippines. The Spider Cyclone Ride malfunctioned, which caused some large pieces to fly into the air, and then a couple of the seats actually bounced against the bottom of the ride.
It looked like it was going to be tragic, but fortunately the ride was stopped pretty quickly, and no one was seriously injured.
